Support Coordination
When your NDIS plan includes
Coordination of Supports, we will provide experienced Support Coordinators who will ensure that you get the most out of your NDIS entitlements.
We support you by:
- Guiding you through the NDIS process. We’ll work with you to identify your goals and select the best services to meet your needs.
- Helping you budget for each type of support, as well as negotiating prices and service agreements.
- Discussing the feasibility of in-home care respite, when required
- Advising you during NDIS plan reviews
- Navigating and optimising your yearly plan, and NDIS approved budget.
- Assisting with Housing Pathways
- Being there for you in times of crisis
- Providing bilingual carers and support workers. On top of mandatory English fluency, Devote Care maintains a roster of employees fluent in Arabic, Farsi or Kurdish
Our team is experienced in managing referrals and proactively supporting you during Physiotherapy, exercise physiologist, occupational therapy, dietician, speech pathology, registered nurses, psychotherapist, and social worker appointments.
Transport and Travel Assist
Whether it’s a day trip to the beach, a weekend getaway, or even just a trip to the local park. Devote Care provides Independent Support Workers who can ensure that you have access to comprehensive support when you want to get out of the house. From booking and packing to going with you on your travels, our carers can help you live a life defined by adventure.
Our transport service enables our participants to access disability supports outside their home, and to facilitate transport arrangements that allow you to achieve the goals set out in your personalised plan.
Domestic Assistance
A clean and tidy home is a place where you can feel safe, comfortable and happy, so we take care of difficult or uncomfortable household duties for you. Cleaning, gardening, cooking, ironing and a whole host of domestic duties are well within our purview. This allows you to focus on what’s most important; yourself and your well-being.
Assistance with Daily Life Tasks
In this category support revolves around helping NDIS participants thrive in a shared living environment, whether the arrangement is temporary or long term. Participants will be assisted and/or supervised as they complete day to day activities at home, helping them to live their lives as independently and autonomously as possible.
This service includes:
- One-to-one assistance with a carer
- Assistance with daily tasks for independent living
- Support for self-care, accommodation and activities within a residency
- Providing short-term residency
- Arranging housing for transitioning into independent living
High Intensity Activity Support
Our team of skilled professionals delivers high intensity supports to NDIS participants, to carry out high intense daily personal activities like Complex Bowel Care, Enteral Feeding/Management, Tracheostomy Management, Urinary Catheter Management, Subcutaneous Injections, Complex Wound Management, Ventilator Management, and Day-to-day management of medications.
Community Nursing Care
Community Nursing Care is provided to participants with complex individual support needs that are best met by someone who has medical training.
Devote Care is a Registered NDIS Provider for Community Nursing Care, delivering:
- Complex Bowel Care
- Enteral (Naso-Gastric Tube – Jejunum or Duodenum) Feeding/Management
- Tracheostomy Management
- Urinary Catheter Management (In-dwelling Urinary Catheter, In-out Catheter, Suprapubic Catheter)
- Subcutaneous Injections
- Complex Wound Management
- Ventilator Management
- Day-to-day management of medications
Devote Care offers two levels of community nursing care: Standard and Higher Intensity Support.
Standard Support can be provided by any properly qualified support worker.
While Standard Support can be provided by any properly qualified nursing professional, Higher Intensity Support generally necessitates specialised, highly skilled, and highly experienced nurses to deliver support to the participant. The Higher Intensity Support is itself tiered in to a three-level intensity structure– all based on the skill and qualifications of the staff members used to deliver support to participants. As a result of the nature of this level of support, the associated costs are generally higher.
An individual may require Higher Intensity support when they require:
- Frequent (at least 1 instance per shift) assistance to manage challenging behaviours that require intensive positive behavioural support and/or,
- Continual active support due to complex medical needs (such as unstable seizure activity or respiratory issues).
Community Participation and Social Support
We all feel better when we’re connected with our families and community, whatever that means to us subjectively. For some, it might mean going to church on Sunday and for others, it might be learning a new skill, or maybe even a picnic with our brothers and sisters. Just because your mobility is impacted, or your health isn’t what it could be, doesn’t mean you have to miss out. We strongly believe that community participation helps realise the potential for physical, social, emotional and intellectual development. Devote Care can support you in living a busy life by providing you with adequate and proper motor vehicle transportation to assist you to travel to and from your designated activity place or wherever you want to go.

- Community centre
- Eating out
Devote Care will ensure that you have a support worker, qualified to provide the level of support your individual needs necessitate, to assist you.
Additionally, we regularly organise a range of activities and programs aimed to assist participants in leading a full and happy life. Social inclusion and interaction programs are specially designed to meet every individual’s need and to assist them in finding their “special place” in life, where they feel valued and comfortable within their surroundings.
Assist with Life Stage Transition
We provide quality assistance to all participants, to be able to adapt and manage transitions in life stages. Our pro-active approach allows the participants to coordinate with the transitions, as it certainly strengthens their ability for the same. We always assist you (participant), whilst following a short and long-term support plan and enable you to be an active member of the community. We offer assistance with decision making, budgeting and planning, skill development and in other areas of transition.
Independent Living
Supported Independent Living (SIL) is provided for participants who need help with and/or supervision of their daily tasks. The goal of this service is to support the participant in developing their skills to live as independently as possible.
There’s always something that needs to be done so it’s comforting to know you can get help with these functions and maintain your independence. You may need help with transport to go out and perform errands like visiting the bank, going to the doctor or picking up your new glasses. Your Independent Support Worker can even do your shopping for you and put your groceries away. Devote care has a solution for all the little – and not so little – tasks and duties
Group Based Centre Activities
Support Centre Based Activities and Groups provide opportunities for our participants to socialise. Participants are welcome to undertake activities independently, participate in facilitated workshops or a combination of both.
These activities are highly individualised, and we encourage our participants to get actively involved.
